What Is PT-141?
PT-141, also known as bremelanotide, is a synthetic peptide that activates melanocortin receptors in the brain. These receptors play a role in sexual desire, motivation, and arousal.
Researchers actually discovered PT-141 by accident. While studying a different peptide designed to increase skin pigmentation, participants unexpectedly reported increased sexual arousal! That finding eventually led to the development of bremelanotide.
Today, the FDA approves bremelanotide under the brand name Vyleesi® for premenopausal women with acquired, generalized hypoactive sexual desire disorder (HSDD).
Compounded PT-141 is also prescribed off label for postmenopausal women and for men who feel that low desire has become a barrier to a healthy, satisfying sex life. Off-label prescribing is common in medicine and gives providers the flexibility to personalize treatment based on each patient’s symptoms and goals.
How PT-141 Works
Scientists don’t completely understand exactly how PT-141 works, but they believe it acts on pathways in the brain involved in sexual desire. Even though we’re still learning about its precise mechanism, PT-141 has been studied in clinical trials and has an established safety profile.
Unlike medications like Viagra® or Cialis®, which improve blood flow, PT-141 works in the brain.
There’s an old saying that the brain is the body’s most important sexual organ. While that’s a simplification, there’s a lot of truth to it. Sexual desire often starts between the ears long before it happens between the legs.
That’s why PT-141 may help people who have lost interest in sex, even if their physical function is otherwise normal. It can also be used alongside other treatments when both desire and physical performance need support.
PT-141 vs. Viagra® and Cialis®
PT-141 is often compared with sildenafil (Viagra®) and tadalafil (Cialis®), but they treat completely different conditions.

For some people, one medicine is enough. Others benefit from both kinds. For example, a man with erectile dysfunction and low libido may take tadalafil to improve erections while using PT-141 to help restore sexual desire. Because they work in different ways, they often complement each other rather than compete.
PT-141 for Women
Low sexual desire is one of the most common sexual health concerns among women.
Hormonal changes during perimenopause and menopause certainly contribute, but stress, medications, relationships, and other health conditions certainly can also play a role.
PT-141 may be worth considering if you:
- Have lost interest in sex and find it distressing
- Have optimized your hormones but still have low libido
- Feel physically capable of intimacy but simply aren’t interested
- Developed low desire while taking an SSRI antidepressant
- Have worked on intimacy barriers in therapy and still experience low desire
Most women don’t describe PT-141 as creating an overwhelming sexual urge. Instead, many say they feel more receptive to intimacy. Interest comes more naturally, physical sensations feel stronger, and initiating intimacy feels easier than before.
Like any medication, PT-141 doesn’t work for everyone.
Some patients notice a meaningful improvement after the first few uses, while others experience little benefit. That’s why we typically recommend trying it several times before deciding whether it’s the right fit for you.
Does PT-141 Replace Hormone Therapy?
Usually not.
Low libido rarely has a single cause. Hormones, stress, medications, sleep, relationship health, and underlying medical conditions can all play a role.
