11 Things to Know Before Taking Tadalafil or Sildenafil

Erectile dysfunction (ED) affects millions of men, yet many wait months or even years before talking to a healthcare provider. Don’t let that be you.

While tadalafil and sildenafil work in similar ways, there are some important differences that can help determine which one is the better fit for your lifestyle.

Here’s what you should know before starting either medication.

1. They Work the Same Way, but They’re Not Exactly the Same

Tadalafil and sildenafil both belong to a group of medications called PDE5 inhibitors.

They work by relaxing blood vessels and increasing blood flow to the penis during sexual stimulation, making it easier to achieve and maintain an erection.

The biggest difference isn’t how they work. It’s how long they work.

2. Tadalafil Lasts Much Longer

This is one of the biggest reasons many men choose tadalafil.

Sildenafil typically works for about 4 to 6 hours, which is why it’s sometimes called the “date night pill.” You take it shortly before sexual activity, and its effects wear off later that day.

Tadalafil can remain effective for up to 36 hours, earning it the nickname “the weekend pill.” That longer window allows for much more spontaneity, without feeling like intimacy has to happen on a strict schedule.

That doesn’t mean you’ll have an erection for 36 hours. It simply means your body is more responsive to sexual stimulation during that time.

Many couples prefer tadalafil because it takes the pressure off the clock and allows intimacy to happen more naturally.

3. Neither Medication Increases Your Sex Drive

This is probably the biggest misconception about ED medications.

Tadalafil and sildenafil improve blood flow. They do not create sexual desire. (This does, though!)

You still need to be mentally interested and sexually stimulated for the medication to work.

If your biggest concern is a low sex drive rather than difficulty getting an erection, hormones, stress, medications, poor sleep, or other health conditions may be playing a larger role.

4. ED Is Often a Symptom, Not the Problem

It’s easy to think erectile dysfunction is simply part of getting older.

Sometimes it is. More often, it’s a sign that something else deserves attention.

Common contributors include:

  • Low testosterone
  • Diabetes or prediabetes
  • High blood pressure
  • High cholesterol
  • Obesity
  • Poor sleep
  • Sleep apnea
  • Depression or anxiety
  • Certain prescription medications
  • Smoking

5. Timing Matters

Although both medications work well, they have different timing recommendations.

Daily tadalafil may also improve urinary symptoms related to an enlarged prostate (BPH), making it a good option for men who have both conditions.

7. Side Effects Are Usually Mild

Most men tolerate these medications well. There’s a reason healthcare providers write more than 10 million prescriptions for these medications every year! For most men, the benefits far outweigh the side effects.

The most common side effects include:

  • Headache
  • Facial flushing
  • Nasal congestion
  • Upset stomach
  • Heartburn

Tadalafil is also slightly more likely to cause muscle aches or back pain because it stays in the body longer.

Most side effects improve as the medication wears off.

8. They Aren’t Safe for Everyone

Although these medications are safe for many healthy men, they’re not appropriate for everyone.

You should not take tadalafil or sildenafil if you:

  • Use nitrate medications for chest pain
  • Use recreational nitrites (“poppers”)
  • Have certain serious heart conditions where sexual activity is not recommended
  • Have very low blood pressure

That’s why a medical evaluation is important before starting treatment.

9. Hormones May Be Part of the Picture

If erectile dysfunction is happening alongside symptoms like low libido, fatigue, loss of muscle, decreased motivation, or mood changes, low testosterone may also be contributing.

ED medications aren’t a substitute for good health.

  • Regular exercise
  • Maintaining a healthy weight
  • Eating enough protein
  • Prioritizing sleep
  • Limiting alcohol
  • Managing stress
  • Controlling blood sugar and blood pressure
  • Avoiding tobacco products

There’s no single “best” ED medication.

Remember, though, that these medications improve erections, but they don’t treat the underlying cause of erectile dysfunction. Lasting improvements often come from addressing issues like low testosterone, diabetes, poor sleep, excess weight, or cardiovascular health alongside medication.
