Medicare GLP-1 Bridge Program: Do You Qualify in 2026?

Beginning July 1, 2026, Medicare beneficiaries may qualify for the new Medicare GLP-1 Bridge Program, which provides access to certain GLP-1 weight loss medications for a reduced monthly cost. Eligibility is based on your BMI, medical history, and enrollment in a Medicare Part D prescription drug plan. Body mass index (BMI) must be ≥35 prior to starting GLP-1 therapy to qualify. Medicare patients with significant other medical problems may also qualify.

What Is the Medicare GLP-1 Bridge Program?

For years, Medicare generally excluded medications prescribed solely for weight loss. As obesity rates increased and research demonstrated the health benefits of GLP-1 medications, pressure grew to improve access for Medicare beneficiaries.

Beginning July 1, 2026, Medicare is launching the Medicare GLP-1 Bridge Program, a temporary demonstration program designed to expand access to obesity treatment medications for qualifying beneficiaries.

The program is currently scheduled to run through December 31, 2027.

For many patients, this may represent the first opportunity to obtain GLP-1 treatment through a Medicare-supported pathway.

Which Medications Are Included?

Current medications included in the Bridge Program may include:

  • Wegovy
  • Oral Wegovy
  • Zepbound
  • Foundayo

Medication availability and coverage rules may change as CMS updates the program.

Who Qualifies for the Medicare GLP-1 Bridge Program?

To qualify, patients must:

  • Eligible for Medicare. Typically this is for patients over 65 years old
  • Be enrolled in a Medicare Part D prescription drug plan
  • Meet BMI and medical qualification requirements

Unlike previous Medicare coverage pathways, qualification is largely based on BMI and associated medical conditions.

Body Mass Index (BMI) 35 or Higher

Patients with a body mass index (BMI) of 35 or greater may qualify based on obesity alone when participating in ongoing lifestyle modification efforts.

This category is expected to include many patients with severe obesity who previously had limited access to GLP-1 treatment through Medicare.

Example

A patient with:

  • BMI of 38
  • No diabetes
  • No cardiovascular disease

May still qualify under this category.

  • BMI 35+ may qualify
  • BMI 30+ with heart failure, uncontrolled hypertension, or chronic kidney disease
  • BMI 27+ with pre-diabetes, prior heart attack, stroke, or symptomatic peripheral artery disease

BMI 30 or Higher

Patients with a BMI of 30 or greater may qualify if they also have one of the following conditions:

  • Heart failure
  • Uncontrolled hypertension
  • Chronic kidney disease

Example

A patient with:

  • BMI of 31
  • Chronic kidney disease

May qualify even if they do not have diabetes or previous cardiovascular events.

BMI 27 or Higher

Patients with a BMI of 27 or greater may qualify if they have:

  • Pre-diabetes
  • Previous heart attack
  • Previous stroke
  • Symptomatic peripheral artery disease

This category allows patients with lower BMI levels but significant cardiovascular risk factors to access treatment.

Example

A patient with:

May qualify for the program.

Why This Matters

Obesity is now recognized as a chronic medical condition that contributes to:

  • Heart disease
  • Stroke
  • Diabetes
  • Sleep apnea
  • Joint pain
  • Reduced quality of life

GLP-1 medications have demonstrated significant benefits beyond weight loss, including improvements in metabolic health and cardiovascular outcomes.

The Medicare GLP-1 Bridge Program represents one of the largest expansions of obesity treatment access for seniors to date.
