GLP-1 medications have changed the way we treat obesity. What started as a niche treatment has quickly become one of the most widely used prescription therapies in America.
But there’s another trend happening at the same time.
While brand-name medications like Zepbound®, Wegovy®, Ozempic®, and Mounjaro® still make up most prescriptions, more patients are choosing compounded GLP-1 medications instead.
Why?
For many people, it comes down to three things: cost, availability, and flexibility.
GLP-1 Use Has Nearly Quadrupled
According to a 2026 Gallup survey of more than 5,000 U.S. adults:
- 11% of American adults are currently using a GLP-1 for weight loss, up from just 3% in 2024.
- 15% have used one at some point.
- Awareness of GLP-1 medications has climbed to 91% of U.S. adults.
As GLP-1 use has increased, Gallup also found that the U.S. obesity rate has fallen from 39.9% in 2022 to 36.4% in 2026, while diabetes rates have leveled off after years of increasing.
While many factors influence these trends, it’s another sign that GLP-1 therapy is becoming part of mainstream obesity treatment.

More Patients Are Moving Toward Compounded GLP-1s
Brand-name medications still account for most current users.
However, Gallup found something interesting:
- 35% of compounded GLP-1 users previously used a brand-name medication.
- While only 10% of brand-name users switched from compounded therapy.
In other words, the movement is largely one direction: patients are leaving brand-name medications for compounded versions.

Among patients who switched from brand-name medications to compounded GLP-1s:
- 66% said cost or insurance coverage was the primary reason.
That probably isn’t surprising.
Many insurance plans don’t cover GLP-1 medications for weight loss, and even when they do, deductibles and copays can be substantial.
A 2024 review published in JAMA (Journal of the American Medical Association) concluded that the high cost of anti-obesity medications remains one of the biggest barriers to treatment access and long-term health equity, leaving many patients unable to start or continue therapy despite clear medical benefit.

Compounded medications often provide a more affordable cash-pay option that allows patients to stay on treatment longer.
For many people, the best medication is the one they can realistically continue taking.
Please note that compounded medications must be prescribed by a licensed health care clinician and have a valid reason for requiring a compounded medication. Price of medication is not a valid clinical indication.
Patients Report Similar Results
One of the more interesting findings from the Gallup survey involved patient satisfaction.
Both groups reported very similar effectiveness:
- 77% of compounded users said their medication was effective or extremely effective.
- 74% of brand-name users reported the same.
Even more notable:
- 39% of compounded users described their medication as extremely effective, compared with 32% of brand-name users.
It’s important to understand what this does—and doesn’t—mean.
This was a patient survey, not a randomized clinical trial. These responses reflect patient experiences rather than direct scientific comparisons between medications.
Still, it’s encouraging that patients using compounded medications reported outcomes that were at least comparable to those using brand-name products.
Please note that compounded medication is not FDA approved and reported study results for brand name medications do not include compounded medications.
Why Many Patients Prefer Compounding

For many people, those advantages make it easier to stay consistent, and consistency is one of the biggest predictors of long-term success.
Personalized Dosing Is One of the Biggest Advantages
One benefit of compounded GLP-1 medications that often gets overlooked is the ability to personalize dosing.
Brand-name GLP-1 medications are only available in a handful of fixed-doses. That works well for many patients, but not everyone responds the same way.
Some people lose weight quickly on a lower dose and never need to increase it. Others experience nausea or constipation when moving up to the next standard dose. Some hit a plateau and benefit from smaller adjustments rather than a large jump.
With compounded medications, providers can often make more gradual dose adjustments instead of being limited to the manufacturer’s preset dosing schedule.

Safety Matters More Than Price
Cost should never be the only consideration.
Not all compounded medications are created equally.
When compounded GLP-1 therapy is appropriate, it’s important to obtain medication from reputable FDA-registered 503 compounding pharmacies that follow strict quality standards and provide ongoing testing.
Patients should also work with an experienced medical provider who monitors:
- Side effects
- Weight loss rate
- Muscle preservation
- Nutrition
- Laboratory values
- Long-term maintenance planning
The medication is only one part of a successful treatment plan.

The above data highlights an important reality.
Patients aren’t necessarily looking for a specific brand name. They’re looking for a treatment they can afford, tolerate, and continue long enough to improve their health.
