Which Peptides Actually Work for Weight Loss?

Search “best peptides for weight loss” and you’ll find dozens of articles listing every peptide imaginable.

The problem? Most of them aren’t actually weight loss medications.

Some may support recovery, muscle preservation, or healthy aging. Others are still being studied. Only a few have strong evidence showing they help people lose a meaningful amount of body fat.

As a medical provider, I rank peptide therapies based on three things:

  • How well they work
  • How strong the scientific evidence is
  • Whether they help patients achieve lasting improvements in health

Here’s how I currently rank them.

1. Tirzepatide ★★★★★

The current leader for medical weight loss

If someone asks me which peptide-based medication is most effective for weight loss today, my answer is usually tirzepatide.

Unlike older medications, tirzepatide activates both the GLP-1 and GIP receptors. Together, these hormones reduce hunger, improve fullness, decrease food cravings, and help regulate blood sugar.

In the SURMOUNT-1 trial, participants lost an average of 20.9% of their body weight after 72 weeks on the highest dose.

That’s among the greatest weight loss ever seen with a prescription medication!

Beyond the scale, many patients also experience improvements in blood pressure, cholesterol, blood sugar, sleep apnea, fatty liver disease, and quality of life.

For most patients, tirzepatide remains my first choice.

2. Semaglutide ★★★★☆

An outstanding option with years of proven success

Semaglutide transformed obesity medicine.

It works by mimicking GLP-1, one of your body’s natural hormones that regulates appetite and fullness. Most people eat less without constantly feeling hungry.

Clinical trials have shown average weight loss of approximately 15% of starting body weight.

So why four stars instead of five?

Simply because tirzepatide generally produces greater average weight loss with fewer side effects.

That doesn’t make semaglutide a second-rate option. Millions of people have successfully lost weight with it, and it remains one of the most effective treatments available.

3. Sermorelin ★★★☆☆

Not a weight loss medication, but it may support better results

Sermorelin doesn’t directly help people lose weight.

Instead, it stimulates your body’s natural production of growth hormone.

So why is it on this list?

Because successful weight loss isn’t just about losing pounds. It’s about preserving muscle while losing fat.

For appropriately selected patients, sermorelin may support better recovery, healthier sleep, and maintenance of lean muscle mass. Those benefits may make it easier to stay active and maintain body composition throughout a weight loss journey.

I don’t recommend sermorelin instead of GLP-1 therapy. But often I do recommend it alongside a comprehensive wellness plan when preserving muscle is an important goal.

4. What About Retatrutide? ★★★★★???

The peptide everyone is talking about

Retatrutide has generated tremendous excitement, and for good reason.

Early clinical trials have shown even greater weight loss than today’s leading medications. That’s impressive.

It’s also early.

While the initial research looks extremely promising, retatrutide is still being studied. I’d like to see additional long-term safety data and more real-world experience before recommending it over medications we already know work well and are safe.

Could it eventually become the best weight loss peptide available?

Absolutely.

It just isn’t there yet.
