Constipation While Taking GLP-1 Medications

Managing Constipation While Taking GLP-1 Medications

For many patients on GLP-1 medications, constipation can become an uncomfortable side effect, especially as the dosage increases. While these medications are highly effective for weight loss and managing diabetes, they can affect the digestive system in various ways. Here at Flow Wellness, we want to help you understand why these symptoms occur and how you can manage them effectively.

Digestive Changes with GLP-1 Medications

During the first few months of treatment, many patients may experience changes in their bowel movements, including both constipation and diarrhea. However, constipation tends to become more common as patients progress to higher doses of GLP-1 medications. This can happen because these medications slow down gastric emptying, reducing how fast food moves through the digestive system.

Strategies to Prevent and Manage Constipation

If you’re experiencing constipation, don’t worry—there are several strategies that can help keep you regular and comfortable:

  • Increase Fiber Intake:
One of the best ways to prevent constipation is by increasing your daily fiber intake. Fiber helps bulk up the stool and makes it easier to pass through the digestive system. Be sure to drink plenty of water to help the fiber work effectively. Total dietary intake should be at least 30 grams per day.
    • Some foods that are high in fiber include: beans and peas, vegetables, fruits, whole grains, nuts, chickpeas, quinoa, and avocados.
    • Fiber Supplements
      • In addition to increased fiber intake with diet, we recommend taking psyllium fiber capsule (Three capsules = 1 gram of fiber) or a similar fiber supplement such as Metamucil (One teaspoon = 5 grams of fiber).
      • Fiber gummies also help. Three gummies have 7 grams of fiber.
  • Add Miralax:
If you find that fiber is not enough, you can add Miralax (polyethylene glycol) to your regimen. Take this according to package instructions.
  • Use Senna if needed:
If you are not having a daily bowel movement, adding a senna supplement can help. Senna is a natural stimulant laxative that can be taken in addition to fiber and miralax.  If taking for more than 2 weeks, please mention to your clinician.

Stay Ahead of Digestive Issues

While constipation can be an uncomfortable side effect of GLP-1 medications, following these recommendations can help you stay regular and prevent symptoms from interfering with your progress. Remember to speak with your healthcare provider if your constipation is persistent or if you’re unsure about adding any new supplements to your routine.

At Flow Wellness, we’re here to support you every step of the way, ensuring that you can achieve your health and weight loss goals while feeling your best.

Frequently Asked Questions About Constipation While Taking GLP-1 Medications

Is constipation a common side effect of GLP-1 medications?

Yes. Constipation is one of the most common digestive side effects associated with GLP-1 medications such as tirzepatide and semaglutide. These medications slow stomach emptying and can reduce overall bowel motility. At Flow Wellness, we help patients proactively manage constipation so they can stay comfortable while continuing their treatment plan.

Why do GLP-1 medications cause constipation?

GLP-1 medications work in part by slowing digestion and reducing appetite. While these effects can support weight loss, they may also cause food to move more slowly through the digestive tract. Flow Wellness educates patients on hydration, nutrition, and medication adjustments that may help minimize these symptoms.

When does constipation usually start after beginning a GLP-1 medication?

Many patients notice constipation within the first few weeks of treatment or after a dose increase. Symptoms often improve as the body adjusts. The Flow Wellness team monitors patients closely during dose changes and can recommend strategies to improve bowel regularity.

How much water should I drink while taking a GLP-1 medication?

Adequate hydration is essential. Many patients benefit from drinking at least 64–100 ounces of fluid daily, depending on body size, activity level, and medical conditions. Flow Wellness frequently finds that increasing hydration is one of the simplest and most effective ways to reduce constipation.

What foods can help prevent constipation on GLP-1 medications?

High-fiber foods such as vegetables, berries, beans, legumes, chia seeds, flaxseeds, and whole grains may help support healthy digestion. Flow Wellness also encourages patients to prioritize protein while gradually increasing fiber intake to avoid worsening digestive symptoms.

Can fiber supplements help with GLP-1-related constipation?

For many patients, yes. Products containing psyllium husk, partially hydrolyzed guar gum, or other fiber blends may help improve bowel regularity. Flow Wellness can help determine whether a fiber supplement is appropriate based on your symptoms and treatment plan.

What supplements does Flow Wellness commonly recommend for constipation?

Recommendations vary by patient, but common options may include magnesium, fiber supplements, increased hydration, and certain digestive support products. Flow Wellness providers evaluate each patient’s symptoms and medical history before recommending any supplement regimen.

Does exercise help with constipation while taking GLP-1 medications?

Yes. Regular physical activity can stimulate bowel movements and improve overall digestive function. Flow Wellness encourages patients to incorporate walking, resistance training, and other forms of exercise into their weight loss program whenever possible.

Should I lower my GLP-1 dose if I become constipated?

Not necessarily. Mild constipation can often be managed without changing your medication dose. However, if symptoms become severe or persistent, Flow Wellness may recommend adjustments to your treatment plan after a medical evaluation.

When should I contact my provider about constipation?

You should contact Flow Wellness if constipation is severe, lasts more than several days despite treatment, is accompanied by significant abdominal pain, vomiting, inability to pass gas, abdominal swelling, or any other concerning symptoms. These symptoms may require prompt medical evaluation.

Can semaglutide and tirzepatide both cause constipation?

Yes. Both semaglutide and tirzepatide can cause constipation because they affect digestion and stomach emptying. Some patients tolerate one medication better than another, and Flow Wellness can help determine which option may be the best fit for your goals and side effect profile.

How does Flow Wellness help patients manage GLP-1 side effects?

Flow Wellness provides personalized medical weight loss support that includes medication guidance, side effect management, nutrition counseling, body composition scans, and ongoing provider access. Our goal is to help patients achieve successful weight loss while minimizing uncomfortable symptoms such as constipation.

Does constipation mean the medication is working?

Not necessarily. While slowed digestion is part of how GLP-1 medications work, constipation itself is a side effect rather than a sign that the medication is more effective. Flow Wellness focuses on healthy, sustainable weight loss while helping patients maintain good digestive health.

Can I continue taking my GLP-1 medication if I am constipated?

Many patients can continue treatment while implementing hydration, dietary, exercise, and supplement strategies. If constipation becomes severe or does not improve, the Flow Wellness team can review your treatment plan and recommend appropriate next steps.
